Sports Physicals For All Ages
Many people have to have an annual work physical to keep their insurance, or to participate in activities such as school sports. We are happy to comply with these requirements. Our physical sports exams are the lowest cost in the metro area and meet all the requirements for OSSAA or other leagues.
Generally speaking, we will review your medical history, conduct a visual evaluation, and refer you for additional testing and diagnosis as needed. Following the session, your Family First provider will prepare a completed medical examination report form as required by your league.
Sports Physicals are provided for participants of all ages. We provide school, club, and team physicals for all participants. You’ll meet with one of our experienced providers and get your paperwork completed in just one trip. Bring your forms and let us get you back in the game, for just $25.00. Please allow about 30 minutes to complete the exam.
